Please advise members of your party to wear appropriate gym attire. Close fitting clothing should be worn. Avoid blue jeans and skirts and always choose clothing without snaps, buttons and/or zippers.

Parents of the Birthday Child may be in the STRONG BY JUMPING to take pictures or to video tape the party. However, NO parents are allowed on any of the equipment, including the trampoline without staff permission .

Please help our staff stay on schedule by arriving 15 minutes early for the party and assisting us in getting your party ready.

The behavior of the children participating in the party is the responsibility of the host/hostess. Parents/adults are responsible for the supervision of their children all the time including food time.

The undersigned, herein know as the applicant, understand and agrees that he/she or organization that he/she represent shall assume all risk for loss, damage, liability, injury, cost or expense that may occur during or a results of use listed above, the applicant further agrees that in consideration of permission to use STRONG BY JUMPING property above, he/she and the organization will defend, indemnify, and hold harmless STRONG BY JUMPING and STRONG BY JUMPING employees and agents from and against all claims, liability, damages, and or injuries to persons and property, (including demands, losses, actions, causes of actions, demands, liability, expenses, charges, assessments, fines or penalties of any kind, and property. The applicant further agrees to be personally responsibility for any damage sustained to the grounds, building, fixtures or equipment, as a results of their use of STRONG BY JUMPING property. The applicant further certifies that he has read (or interpreted), understand and agrees to abide by any and all attached reservation policies of STRONG BY JUMPING.

Participants involved any activities offered by STRONG BY JUMPING may be photographed or videotaped during training. The undersigned hereby consents to the use of these photographs and/or videos without consent or compensation on the STRONG BY JUMPING website or in any editorial, promotional or advertising material produced by STRONG BY JUMPING.

CCTV cameras are in operation.
Images are being monitored and recorded for the purpose of crime-prevention,the prevention of anti-social behaviour, the prevention of bullying, for the safety of our staff, kids and jumping members and for the protection of world jumping clonmel property.
All material recording will only be for internal access and will not be accessed by the public.
This system will be in operation 24 hours a day, every day.
These images may be passed onto An Garda Siochána.

I understand that each guest attending the party must have a signed waiver on file, and that my deposit is non-refundable. I have read the Birthday Party Rules and agree to abide by them as well as the terms stated in this agreement.

By signing this form applicat agrees to the terms and conditions listed in the policy which is made part of this agreement.
